- Medical card and players medical information
- Copy of players birth certificate
- 1" x 1" photo of the player with their name legibly written on the back
- Payment: (can be deferred until 1/16/09 5:00 PM)
- Payment at the time of registration would be appreciated
Please register on-time so we can get our teams organized.
Incomplete registrations after 1/16/09 5:00 PM will be considered CANCELLED.
- Missing supporting documents include:
- Birth Certificate
- COMPLETED & SIGNED Medical Release Form
- FULL Payment
- 1" x1" picture of the player is NOT REQUIRED as a supporting document. However, without this picture your player will not receive a player CARD required by MYSA and will not be able to participate in practices, games and/or tournaments.
We are requesting earlier registration in order to determine the number of teams the Red Wing Soccer Club will field for the summer 2009 season. If you do not register by 11/30/08 it may be possible that a team will not be formed for your player to be on due to a lack of players to form a team. Teams will be formed 12/4/08. After this date only open roster positions on formed teams will be filled.
Why is team formation so early?
- This extra time will hopefully be enough to find coaches and managers for the teams.
- This will allow time for uniforms to get ordered and back to the club in time for the season.
- The teams will have time to get organized:
- determine practice schedules
- select tournament(s)
- review vacation schedules
- other issues, etc.
- When schedules are released by MYSA to the clubs any schedule conflicts can be immediately addressed by each teams contacts.
This is all in an effort to minimize any impact on our team volunteers and most importantly the players and parents.
If you are considering playing summer 2009 soccer please register by 11/30/08. If you change your mind between 11/30/08 and 1/16/09 5:00PM you can cancel your registration without any penalties. We need to know who is interested so we can find coaches and managers for the teams.
